ANNOUNCEMENT!

Document acceptance for clinical residency for 2019-2020 academic year has started in Andijan state medical institute.

Acceptance will last from July 22 to August 20, 2019.

The following documents are required:

(FOR THE INSTITUTE’S GRADUATES)

1. An application addressed to the head of the institution for participation in the clinical residency contest;
2. Personal form of the employee with photo (unauthorized);
3. The biography;
4. Passport, diploma, attachment to diploma with grades, original and copy of the military certificate;
5. Photo (for photopassport) 6 pieces (3,5*4,5);
6. Medical certificate ( form 086). Medical examination must include a psychiatrist’s and narcologist’s medical reports.
7. Permission from marketing department.
8. 20 sheets of paper.
9. A folder and 2 envelops to put documents.

 

FOR THE PHYSICIANS WITH EXPERIENCE

1.An application addressed to the head of the institution for participation in the clinical residency contest;

2.Personal form of the employee with photo (authorized with official stamp);

3.A testimonial from the last place of work(authorized with official stamp);

4. The biography;

5.Passport, diploma, attachment to diploma with grades, original and copy of the military certificate, original and copy of a work record book(authorized with official stamp);

6.Photo  (for photopassport) 6 pieces (3,5*4,5);

7. Medical certificate ( form 086). Medical examination must include a psychiatrist’s and narcologist’s medical reports.
8. Permission from marketing department.
9. 20 sheets of paper.
10. A folder and 2 envelops to put documents.
