Dear residents admitted to the clinical residency of  Andijan State Medical Institute, for the 2020-2021 academic year!
You will be asked to submit the following documents to the Clinical Residency Department of the Institute from October 12 till October 22, 2020.
FOR INSTITUTE GRADUATES

1. Application for participation in the competition for clinical residency on behalf of the head of the institution;
2. Personnel record sheet with photo (unconfirmed);
3. Biography;
4. Passport, diploma, a sheet with additional grades to the diploma, the original of the military ticket and a copy are handed over to the participants of clinical residency;
5. Photograph (for photo passport) 6 pieces (3,5×4,5);
6. Certificate of medical examination.

FOR DOCTORS WITH WORK EXPERIENCE

1. Application for participation in the competition for clinical residency on behalf of the head of the institution;
2. Personnel record sheet with a photo (approved);
3. Description of the last place of work;
4. Biography;
5. The original passport, diploma, military ID and employment record book of the clinical residency entrants shall be submitted, and a copy shall be submitted with confirmation;
6. Photo 6 pieces (3,5×4,5);
7. Certificate of medical examination
